VO₂max & High-Intensity Exercise: Preserve the Cardiovascular Ceiling

High-intensity cardiorespiratory training asks a different question:

How much work can the cardiovascular system produce when demand becomes high?

VO₂max—the maximum rate at which the body can take in, transport and use oxygen during intense exercise—is one of the principal measurements of this upper capacity.

For longevity, this matters because ordinary activities consume a percentage of our maximum capacity. Climbing four flights of stairs is easy if it requires only a modest proportion of our cardiovascular ceiling. It becomes difficult when age, inactivity or disease have substantially lowered that ceiling.

The objective is therefore not maximal athletic performance.

It is cardiovascular reserve.

Hard does not mean all-out

One of the most important practical distinctions is between high-intensity aerobic intervals and maximal sprinting.

A useful VO₂max interval is usually very hard but repeatable. The objective is not to create the greatest possible suffering during the first repetition.

You should be able to finish the prescribed work interval, recover, and repeat it with reasonably similar quality.

That makes running, cycling and an elliptical trainer particularly practical: the effort can be increased without requiring highly technical movements under fatigue.

Why the Norwegian 4 × 4 became famous

A frequently cited 2007 randomized study from the Norwegian University of Science and Technology compared several endurance-training methods in moderately trained men.

The protocol that later became known as the Norwegian 4 × 4 involved:

4 minutes at approximately 90–95% of maximal heart rate
3 minutes of active recovery
repeated 4 times
.

After eight weeks, the 4 × 4 group increased VO₂max by approximately 7.2%, more than the moderate continuous-training groups in that experiment.1

The study was small and involved moderately trained men, so it should not be treated as proof that 4 × 4 is the universal optimal protocol.

Later research supports the larger principle rather than one magical formula: well-designed intervals can provide a powerful stimulus for increasing cardiorespiratory fitness.

Does HIIT work outside the laboratory?

A useful 2026 systematic review and meta-analysis from Friedrich-Alexander University Erlangen-Nürnberg examined HIIT in real-world rather than tightly controlled laboratory settings.

Thirty-one studies involving 1,119 adults were included. Across the pooled pre-post analyses, HIIT was associated with an average improvement in VO₂max/peak of approximately 3.9 mL/kg/min, together with improvements in several cardiometabolic measures.2

Against active exercise controls, the additional VO₂max advantage was much smaller—about 1.1 mL/kg/min—which is a useful reminder that conventional exercise also works.

The authors also emphasize substantial heterogeneity and only moderate methodological quality.

The correct interpretation is therefore not “HIIT beats everything.”

High intensity provides an efficient additional stimulus, particularly for cardiorespiratory fitness. It complements rather than replaces the aerobic base.

Peter Attia has long included a high-intensity or “Zone 5” component alongside stability, strength and aerobic efficiency, frequently using approximately three- to eight-minute intervals and the 4 × 4 structure as a practical example.3

Andrew Huberman’s discussions with Andy Galpin likewise separate long-duration endurance work from high-intensity cardiovascular training and emphasize balancing the desired adaptation against the fatigue generated by the session.4

David Sinclair’s current 2026 exercise material distinguishes lower-intensity cardiovascular work, harder exercise and resistance training rather than treating exercise as one homogeneous intervention.5

Tim Ferriss has recently discussed using 4 × 4 VO₂max training himself, while his earlier interview with physiologist Martin Gibala and Rhonda Patrick explored shorter interval approaches, covered several options ranging from 4 × 4 intervals to one-minute and shorter formats, and the broader evidence that brief intense efforts can generate significant adaptations.6

The useful convergence is again simpler than the protocols themselves:

occasionally ask the cardiovascular system to operate near the upper end of sustainable aerobic capacity.

Programme 1: the reference 4 × 4

This is the most straightforward longevity reference workout.

Warm-up: 10 minutes, progressing gradually.

Then:

4 minutes hard
3 minutes very easy
repeat 4 times

Finish with 5–10 minutes easy.

Total workout: approximately 40 minutes.

The work intervals should feel roughly 8–9 out of 10: very hard, but paced so that the fourth interval remains achievable.

If the first interval is dramatically faster than the fourth, the opening effort was probably too aggressive.

Programme 2: 6 × 2 minutes

For people who find four-minute intervals psychologically or physically difficult:

10 min warm-up
2 min hard + 2 min easy × 6
5–10 min cool-down

This format works well on an elliptical, bicycle or while running.

Programme 3: introductory intervals

Someone coming from steady aerobic exercise but with little interval experience can begin with:

8–10 min warm-up
1 min hard + 2 min easy × 6
5–10 min cool-down

After several weeks, some intervals can be lengthened toward two, three or four minutes.

There is little reason for a beginner to begin by chasing maximal heart rate.

Best option indoors: elliptical trainer

The elliptical has an important advantage for interval training: cardiovascular demand can become very high without the same impact forces as hard running.

For the 4 × 4:

increase resistance and/or cadence for four minutes, then reduce both substantially for the recovery period.

This can be especially attractive when the weekly programme already includes running or lower-body strength work.

Best outdoor option: bicycle

The bicycle is similarly easy to control.

After a 10-minute warm-up:

4 × 4 minutes on a sustained climb, against wind, or in a sufficiently high gear/resistance, with approximately three minutes of very easy spinning between intervals.

The bicycle also allows the same equipment to serve two very different functions: a long Saturday ride can provide the aerobic base, while Friday intervals train the ceiling.

Running option: controlled hill intervals

Running is highly effective but mechanically more demanding.

A moderate hill can make the cardiovascular system work very hard without requiring extremely high flat-ground speed.

Example:

10 min easy running
3–4 min hard uphill
walk/jog easily for approximately 3 min
repeat 3–4 times
cool down

For runners, this session should be placed sufficiently far from demanding lower-body strength sessions or long runs to allow recovery.

Swimming option

Swimming can also provide high-intensity cardiovascular work for technically competent swimmers.

Because swimming ability varies enormously, duration is often more useful than a universal distance.

For example:

6 × approximately 2 minutes hard but controlled swimming, with easy swimming or rest between repetitions.

If technique deteriorates long before cardiovascular capacity becomes limiting, a bicycle or elliptical trainer is likely a better VO₂max tool.

Is once per week enough?

This requires an important distinction between an optimal VO₂max improvement programme and a balanced longevity programme.

A small 2026 randomized trial in physically active male university students compared 4 × 4 training performed once, twice or three times weekly for six weeks. The two- and three-session groups produced larger VO₂max improvements than the once-weekly group, while the twice-weekly group showed a favourable balance between improvement and adherence.8

But these were young, physically active men specifically training cardiorespiratory fitness. Their programme does not automatically become a longevity prescription for the general population.

A longevity week must also contain strength, movement capacity, aerobic volume and recovery.

We would therefore use:

If a second interval session is introduced, it should generally replace some lower-intensity cardio rather than simply being added to an already full programme.

Safety matters more here

High-intensity exercise imposes substantially greater cardiovascular and musculoskeletal demand than easy aerobic training.

People who are sedentary should build an aerobic base first. Those with known cardiovascular disease, unexplained chest discomfort, unusual breathlessness, fainting, significant rhythm disorders or other relevant medical concerns should obtain appropriate medical guidance before beginning vigorous interval exercise.

For running intervals, orthopedic tolerance matters as well. A bicycle or elliptical trainer can often generate the required cardiovascular demand with less impact.

How it fits into the week

In the example programme:

Friday = one high-intensity session.

This follows Monday and Thursday resistance sessions and precedes the longer Saturday aerobic session.

If the Friday interval session creates too much fatigue for Saturday, move the harder cardio to Tuesday and use Friday for sustainable aerobic exercise instead.

The calendar is a template, not a law.

Conclusion

Foundation: first establish regular movement and sustainable aerobic training.

Next priority: add one deliberately hard cardiovascular session each week.

If VO₂max becomes a specific objective: use a temporary block with two weekly interval sessions if recovery permits.

Usually unnecessary: maximal sprinting, three HIIT sessions every week, or treating exhaustion as proof that the session worked.

The purpose is not to demonstrate toughness. It is to preserve cardiovascular reserve.
